Center for Elders’ Independence PACE Downtown Oakland
The Center for Elders’ Independence PACE Downtown Oakland supports our participants’ medical, social, emotional and nutritional needs. Our care teams include 11 professional disciplines. Every week, they meet in one room to review our participants’ care plans and changes in medical conditions, social engagement and emotional well-being. Although there are never any co-pays for services or medications, CEI-PACE will be the participant’s sole service provider & CEI guarantees access to services, but not to a specific provider.
